Day of Your Visit

Follow your doctor’s instructions to prepare for your procedure or test.

Please bring the following items with you to your appointment:

• Driver’s license/photo ID
• Insurance card
• Referral form (if required)
• Signed physician order (if required)
• Insurance co-payment/deductible

Food and Drink

Some procedures or tests require that you have an empty stomach. Therefore, you may be asked not to eat or drink anything after midnight, including water, gum and candy. Please verify this with your physician prior to your visit.
